By submitting this photo, I hereby grant Empower Society 21, Inc., its representatives, employees, and affiliated partners permission to use the image of my child, as submitted through this form, for the purpose of creating a personalized “It’s Me” Wonderfully Made Coloring Book.

I understand and agree that:

The photo submitted will only be used to create a customized ebook for personal use.

The original photo will not be used in any public promotional materials.

I grant Empower Society 21, Inc. permission to use and showcase the customized coloring book pages (excluding the original image) for promotional, marketing, and educational purposes, including but not limited to social media, website, email campaigns, and printed materials.

I affirm that I am the parent or legal guardian of the child in the submitted photo and have the legal right to grant this permission.
